Thank you for considering a career at Bon Secours!The Supervisor of Care Management is responsible for supervising the day-to-day functions and resources for the Care Management Department. Flexible and willingness to change practices, processes in order to meet the needs of the patients and ministry. Combines the clinical and the financial components to achieve the best possible outcomes for the patients served, as well as the ministryJob Functions:

  • Provides supervision to the Care Management Acute team related to standardized practices and processes such as Advance Care Planning, LOS/LOC management, readmission prevention including use of predictive readmission tool and application of appropriate interventions, denial prevention related to medical necessity through proactive progression of care and addressing barriers (including tracking of avoidable days/delays), daily patient/team rounding, and processes to ensure smooth, safe and seamless transitions.
  • Oversees and facilitates the day-to-day operations of the Care Management Department. Provides coaching and support for staff regarding right care, right time at the right place. Supports and promotes national standards for care management scope of service: Education, Care Coordination, Compliance, Transition Management and Resource Utilization.
  • Utilizes systems within the Care Management Department to support an effective and proactive care management process across the continuum, working closely and aligning with population health efforts and strategies supporting a patient-centered care management model that spans the continuum. Collaborates with ambulatory and post-acute providers/staff to ensure seamless transitions of care.
  • Supports activities related to the Utilization Committee, provides scheduling, agenda, minutes, data or any activity deemed warranted for the preparation and meeting, follow-ups.
  • Supervises daily operations to achieve effective utilization of personnel resources consistent with patient and ministry needs. Supervises development of annual departmental goals and objectives, aligns with ministry CM goals and objectives. Fosters integration and strong collaborative partnerships with nursing, quality, hospitalist, and physician leadership as well as other members of the health care team, ambulatory and post-acute providers. Supports clinical integration activities in order to optimize high quality, cost efficient care.
  • Participates in the development and management of the annual budget. Identifies and achieves cost reductions. Works in concert with ministry colleagues to achieve standardization of operations.
  • Models professional conduct with CM leadership, staff, and all customers. Takes initiative to educate self and staff on the changing healthcare landscape including fee-for-service to value-based models, care model innovations, research and leading evidenced based practices. Promotes and models service excellence behaviors and educates others in service excellence principles. Exhibits flexibility and enthusiasm in style and can adjust to constantly changing economic, technical and strategic shifts. Ensures compliance with federal, state, and local regulations and accreditation requirements impacting care management service
  • Communicates effectively, in a timely manner with all internal and external customers. Serves as a liaison between leadership and staff, physician, patients, and caregivers/families. Collaborates with leadership in the employment performance appraisal process. Meets established deadlines for reports.
  • Recognizes and promotes patient's right to self-determination. Advocates for patients as well as for the ministry.Job Qualifications:Education: Bachelor's Degree in Nursing or Social Work RequiredLicense/Certifications:
    • RN or BSW or above Required
