LPN SHIFT SUPERVISOR: Williamsburg Landing, a CARF-accredited Life Plan Community, seeks a Part-Time LPN Shift Supervisor (11AM - 7PM) to provide leadership for the staff of our Health & Rehab Center to ensure quality care and positive interactions with residents and family members. LPN SHIFT SUPERVISOR JOB DUTIES

• Assists in Skilled Unit admissions, coordinates care for Skilled Unit residents/patients, and prepares discharge paperwork
• Position serves in the Nurse Management on-call rotation along with the Director of Nursing and QAPI/Nurse Educator
• Assists with effective onboarding and training, monitoring performance for regulatory compliance, and addressing staff disciplinary issues; initiates clinical training as needed to ensure resident safety
• Assures information exchange during shift changes, communicating on residents' status and events occurring at the Williamsburg Landing Health and Rehabilitation Center
• Conducts rounds, paying close attention to residents noted on the 24-hour report who have experienced a change in condition; ensures completion of processes and documents resident status or events
• Manages resident and/or family grievances and complaints and reports to the Director of Nursing and the leadership team for follow up via email
• Assists the Director of Nursing of Health & Rehabilitation with the resolution of WeCare Connect feedback from both employees and residents as directed
• Conducts audits and prepares reports as requested by the Director of Nursing
• Ensures that the employee incident procedure for worker's compensation situations is completed thoroughly by the nursing team
• Documents unscheduled absences per policy and initiates attempts to arrange for coverage as needed
• Manages/triages facility issues and takes appropriate action to resolve concerns; informs the Healthcare Administrator, Director of Nursing and appropriate Department Director(s) of issues/actions
• When needed, acts as a floor nurse
• Assists in survey plans of correction as directed by the Director of Nursing
• Completes rounds with visiting providers (i.e. wound doctor, Podiatrist etc.) and follows up appropriately afterwards as directed by the Director of Nursing
• Maintains wound logs
• Maintains both the shower and toilet logs and schedules. Takes appropriate action if not followed up on adequately
• Actively participates in meetings as assigned by the Healthcare Administrator or Director of Nursing
• Communicates frequently throughout their shift with the Director of Nursing and QAPI/Nurse Educator on progress towards assigned tasks
• Participates in the daily stand down meeting with the Director of Nursing, MDS Coordinator, Healthcare Administrator and QAPI/Nurse Educator.
• Participates and contributes to the nursing quality huddle
• Participates and contributes to the monthly all staff meetings
• Performs all other duties as assigned LPN SHIFT SUPERVISOR QUALIFICATIONS
  • Commonwealth of Virginia licensure as a Licensed Practical Nurse
  • A minimum of three (3) years' experience in a post-acute setting and/or medical/surgical nursing
  • One (1) year of nursing supervision preferred
  • Ability to perform duties in accordance with accepted methods and techniques necessary for optimal care
  • Ability to regularly and safely lift a minimum of 35 lbs.
  • Ability to stand and walk for long periods
  • Ability to work on a medication cart for a full shift when needed BENEFITS (Part Time)
