Job Title: Occupational Therapist - SNF/LTCCentra Healthcare Allied is seeking a skilled Occupational Therapist to join our team in a SNF/LTC setting.Job Summary:We are looking for a highly motivated and experienced Occupational Therapist to provide high-quality care to our patients in a SNF/LTC environment. The ideal candidate will have a strong background in occupational therapy and excellent communication skills.Responsibilities:

• Provide occupational therapy services to patients in a SNF/LTC setting
• Develop and implement individualized treatment plans to meet patient needs
• Collaborate with interdisciplinary team members to ensure comprehensive patient care
• Conduct assessments and evaluations to identify patient needs and goals
• Document patient progress and outcomesRequirements:
  • 1 year of recent experience as an Occupational Therapist in a SNF/LTC setting
