Job Description Job Description Salary: 21.00/hour The main duties of the Program Coordinator is to be responsible for the provision of expansion and oversight of Jerolin Management Services community-based community Intellectually Disabled Waiver programs. Duties include, but not limited to, monitoring and supervising staff (to include approving timesheets) and training staff on program specific duties. Completing staff work schedules, Individual Support Plans, Person-Centered Reviews and participating in planning meetings. This postion is also responsible for entering serious incidents and allegations of abse and neglect in the State's CHRIS system, submitting Service Authorizations and accompanying information in the State's WAMS system. The Support Plan Coordinator will work cooperatively in coordinating service delivery with other agencies, departments, etc., in response to the changing needs of the individual. Experience working with Individuals who have an Intelleectual Disability highly preferred. Experience in interpreting Individual Support Plans highly preferred Criminal Background and Central Registry Check required. Valid Driver's License required. Driving Record with points of -6 or higher required. Various trainings required (i.e. CRP/First Aid, Therapeutic Options). A degree in the Human Service field preferred or 5 years experience completing Individual Support Plans and supervising staff who support with individuals who have an intellectual disability.
