Williamsburg Landing, the areas only Five Star CARF-accredited life plan community, has an exciting opportunity for a highly qualified, experienced RN to serve as the Health & Rehabilitation Center's MDS Coordinator. This position manages the RAI process for residents, audits charts to ensure proper documentation and accurate and comprehensive completion of MDS assessments, and initiates baseline care plans for all new admissions. Collaborating with the Health Care Administrator, Director of Nursing, and the Medical Director, the MDS Coordinator chairs the Utilization Review Team and serves as a member of the Quality Assurance Performance Improvement Team, assuring quality outcomes throughout the community.

The RN serving in this role will maintain current knowledge of nursing home licensure and CMS regulations in order to perform clinical record audits of key indicators, analyze data and make recommendations for improving systems and procedures, and advise leadership of noted patterns.

ESSENTIAL JOB FUNCTIONS
Provides the appropriate individuals and/or departments with information necessary to file Medicare billing; is supportive of staff during Medicare audits, as needed

Audits charts to ensure proper documentation and accurate and comprehensive completion of MDS assessments; coordinates the completion of MDS assessments within CMS guidelines and transmits completed MDS's to the state according to CMS submission parameters

Initiates baseline care plans with care plan team for all new admissions per regulations

Reviews notes daily for new residents and weekly for long-term Care residents; prints and scans notes into residents' records

Participates in Interdisciplinary Team (IDT) care plan meetings and resident/family care plan meetings in an organized and efficient manner

Coordinates with the IDT to ensure the accuracy of care plans to include:
Identifying problems/needs of the residents
Indicating the care to be given
Identifying goals to be accomplished
Assigning responsibility to the correct professional service to meet identified goals
Ensures care plans include measurable objectives and timetables
Ensures a comprehensive care plan is completed within seven (7) days of completion of the resident's admission assessment
Reviews and revises care plans with changes in residents' daily routine or plan of care as necessary
Ensures reviews are conducted in a timely manner
Contacts and assists in scheduling participation of outside members of the care plan
Develops Resident Assessments to include:
Develops and completes resident assessments in a timely manner
Reviews and revises MDS as necessary
Completes initial resident assessment per RAI manual regulations
Ensures a complete resident assessment is conducted within 14 days of a significant change in the resident's condition
Ensures each section of the assessment is signed and dated by the person completing that portion of the MDS
Signs and dates the assessment instrument to certify completion
Meets with and solicits advice from charge nurses & department supervisors concerning resident assessment/care plan matters
Communicates to staff when BPCI (Bundled Payments for Care Improvement) admissions arrive and inputs required information into Episode Connect for all BPCI patients

Serves as the Chair and provides effective leadership to the Utilization Review Team; documents meeting decisions; identifies and escalates concerns

Participates in the monthly Triple Check Meeting

Serves as a member of the Quality Assurance Performance Improvement (QAPI) Committee; attends weekly Standards of Care meeting and daily morning meetings

Assists with the collection of data for the monthly COR review

Serves as one of the departmental experts on myUnity EMR and manages user access

Monitors completion of skilled nursing facility certifications and maintains long-term care recertification schedule

Provides assistance during the annual survey and participates in the survey plan of correction as determined by the Healthcare Administrator

Trains staff on documentation expectations and how to utilize EMR

Provides staff training that meets or exceeds mandated standards, develops professional competencies, and prepares them for advancement opportunities

Assists with gathering requested documentation for medical record's requests

Evaluates work processes, policies, staff training needs, and equipment; submits recommendations for continuous quality improvement

Serves as the backup to the Assistant Director of Nursing for worker's compensation during business hours; responsible for ensuring that nurses are educated on handling the worker's compensation process after hours and ensuring that accurate information is submitted timely to Talent Management

Serves in the nurse management call rotation to assist with after-hours questions as well as potential staffing needs

Performs other duties as assigned

QUALIFICATIONS
Current Commonwealth of Virginia Registered Nurse license required; a minimum of two (2) years supervisory experience in a health care setting, preferably long term care with a minimum of one (1) year of experience as an MDS Coordinator, Utilization Review Coordinator, or Medical Records Coordinator; RAC-CT certification preferred

KNOWLEDGE, SKILLS, AND ABILITIES
Knowledge of contemporary nursing practices and procedures, laws, regulations, and guidelines pertaining to long-term care, including Medicare regulations; ability to plan, organize, develop, implement and interpret programs, goals, objectives, policies and procedures necessary for providing quality care; knowledge of general management practices; ability to regularly exercise sound professional judgment; proficiency in Microsoft Office; ability to regularly and safely lift a minimum of 50 pounds; strong patient assessment and patient care skills; ability to make decision independently; strong leadership skills
