Job Description:
Position Purpose: The Envisions Supervisor is charged with
supervising and managing staff and individuals in a habilitative
activity program, which includes balancing the needs of
individuals, families, and other stakeholders. The purpose is to
produce meaningful community and center-based activities, leading
to the integration of people with intellectual disabilities and
related disabilities into community life. This position will ensure
the program is successful in training and providing opportunities
for individuals to enjoy the roles, responsibilities and rewards of
being an active community member/citizen, including establishing
and maintaining relationships with people in the community. Primary
Duties:
- Manage the operations of an assigned team of individuals,
staff, and their involvement in meaningful activities
- Develop and ensure the implementation of individual service
plans, with the person served at the center of the planning
team
- Provide input to, and implement individual behavior management
plans, including staff training of the plans
- Manage assigned staff to ensure the effective application of
individual training methods
- Training and coaching of staff to provide professional,
age-appropriate activities for individuals
- Supervise assigned staff including interviewing, orientation,
training, scheduling, motivating, evaluating, disciplining, and
career development
- Schedule and conduct ISP reviews, within time periods required
by funding and referral sources
- Ensure the appropriate collection of service provision data and
maintain service recipient records/confidential case files
- Maintain individual confidential records files and ensure
program notes and other records are properly maintained by
staff
- Manage the volunteer/community activities, including off-site
quality assurance visits
- Hosts tours by interested individuals/parents and other
community leaders
- Conduct observation studies to ensure active
treatment/participation of individuals in the program
- Provide effective communication with families, other staff,
case managers and other agency personnel to provide exceptional
person-centered services
- Coordinate and support other departments with fulfilling
scheduling/work demands as needed
- Maintains assigned area in a safe, healthful, age-appropriate,
orderly manner and ensures activities are conducted in a safe and
professional manner both on site and in the community
- Participate in department planning and budgeting processes
- Collaborate with other departments to ensure the needs of the
individuals are met
- Comply with federal, state, and other accreditation licensing
standards for the agency/program, to include record keeping,
individual rights, confidentiality, input from persons served,
outcome management reports, as well as all other requested
duties
- May participate as a member of the ID team for the ICF-IID
program
- Serve as a member of the Specially Constituted Committee
- May participate in fundraising/seeking donations for the
program as needed
- Participate on the Incident Review Committee
- Designate or participate on the Safety Committee
- Provide adequate coverage/direct care duties as needed for safe
individual to staff ratio
- Aide families, individuals, care providers and other
stakeholders with resolutions for problem solving
- Monitor and update ISP data information collected for monthly
and quarterly reviews
- Complete quarterly reviews for individuals
- Compare/correct and ensure program data collected coincides
with each other for accurate billing of services/auditing of time
sheets for individuals and staff
- Research, order, and demonstrate the use of age-appropriate
training task materials to the entire Envisions team
- Provide guidance and training to staff that show how to support
individuals for optimum independence and social normalization
- Research and keep abreast of current disabilities and trends in
the field and provide information and training to staff
- Provide support for community integration efforts and HCBS

degree in a related field of study.Experience: Two years'
experience working with people with disabilities or a combination
of education and experience. Environmental: Center based
habilitation environment which includes going out into the
community frequently. May go out in inclement weather such as
extreme heat or cold, rain or snow, out in the warehouse, work out
on the floor with individual, or work behind a desk. May be
required to intervene during behavioral outbursts and perform CPI
techniques. Some walking, standing, bending, carrying of light
items such papers, books, small parts, driving an automobile, etc.
